Internship Request Form

I understand that Riverstone Wellness Collaborative requires: 1) interns to complete both Practicum and Internship requirements over one calendar year, 2) interns to complete clinical sessions from the RWC office, 3) that interns attend a CACREP accredited school, and 4) interns to be fully vaccinated against COVID-19.

RWC will not consider applicants who do not meet the above requirements.
